you for a medical marijuana card?




Human Immunodeficiency Virus (HIV)

Acquired Immune Deficiency Syndrome (AIDS)

Hepatitis CAmyotrophic Lateral Sclerosis (ALS)

Crohn's disease

Agitation of Alzheimer's disease

A chronic or debilitating disease or medical condition or the treatment for a chronic or debilitating disease or medical condition that causes: Cachexia or wasting syndrome; Severe and chronic pain; Severe nausea; Seizures, including those characteristic of epilepsy; Severe or persistent muscle spasms, including those characteristic of multiple sclerosis

Bring your current medical records (diagnosis within the last 12 months) with you at time of appointment or walk-in.

 If you do not have current medical records (diagnosis within the last 12 months) or have not been seen in the last 12 months we can offer an Evaluation for you which requires an additional fee and additional appointment.

$249 New or Renewal Certifications

(Includes State and Filing Fee)

$174 If you have SNAP food stamps 

(SNAP card needs to have your name imprinted on it or award letter needs to have current name and date)

$50 Evaluation Fee 

If you do not have current medical records bring what records you do have with you for our doctor to review and diagnose your condition. You will have a follow-up appointment with another doctor at a later date for your certification. Price becomes $299 including both fees and both appointments.

If you are bringing a signed certification form from another office, please make sure it is filled out entirely and it is the most current ASDHS form

$25 Cannabis Consultations

A separate 30-minute consultation with a Cannabis Consultant to discuss your medicine and how to use it safely. A brief overview of cannabis and it's applications. Q&A included.


For those who cannot leave their home or health care facility we will bring a doctor to see you. 

Simply email us with your medical records and address where we will be seeing the patient and we'll get back to you as soon as our doctor reviews them. Price varies according to distance we travel to you.
