Just bring any current medical records with you

at time of appointment or walk-in.

 If you do not have records or have not been seen in the last 12 months we can offer an Evaluation for you.

Human Immunodeficiency Virus (HIV)

Acquired Immune Deficiency Syndrome (AIDS)

Hepatitis CAmyotrophic Lateral Sclerosis (ALS)

Crohn's disease

Agitation of Alzheimer's disease

A chronic or debilitating disease or medical condition or the treatment for a chronic or debilitating disease or medical condition that causes: Cachexia or wasting syndrome; Severe and chronic pain; Severe nausea; Seizures, including those characteristic of epilepsy; Severe or persistent muscle spasms, including those characteristic of multiple sclerosis

$249 New or Renewal Certifications

$174 if you have SNAP food stamps

(Includes State and Filing Fee)

$50 Evaluation Fee 

(If you do not have current records bring what you do have with you for the evaluation appointment. You will have a follow-up appointment with our doctor at a later date for your actual certification and an additional $50 fee applies.)

$75.00 Discount off State Fee for SNAP patients

(card needs to have your name imprinted on it, award letter needs to have current name and date)

$35.00 Processing/Filing Fee 

(if you are bringing a signed certification form from another office) 

$25 Cannabis Consultations

(A separate 30-minute consultation with a Cannabis Consultant to discuss your medicine and how to use it safely. A brief overview of cannabis and it's applications. Q&A included.)

For those who cannot leave their home or health care facility we will bring a doctor to see you. 

Email us with your medical records and address where we will be seeing the patient and we'll get back to you as soon as our doctor reviews them.
